From what I've read, the Grim Reaper first appeared in the 14th century during the bubonic plague (black death). If you want to be technical the first written record of the Angel of death was actually in Egypt around 3000 years ago. In the first testament. Whether you believe in it or not I beleive it is the first mention of the angle of death.
